To mail or fax your order simply print and complete this form. Product descriptions and pricing can be found on the
individual product pages. Then simply fax the order page to us at: 1-(520)-723-2921 or, if you prefer, mail us the form to the
address above with your payment. Once received our customer service department will process your order and ship accordingly. Some
items may be temporarily out of stock for as long as 2 to 6 weeks due to color, style, size, manufacturer delays or other
